Position Summary:
The Registered Dietitian (RD) works within a team-based environment to provide nutritional expertise to patients over the age of one, and across the lifespan, with the goal of optimizing health through nutrition. The RD will work with a high degree of professional autonomy and judgment in a flexible and responsive manner to assess, plan, implement, and evaluate patient centered nutritional health goals.
Key Responsibilities:
- Works to full scope of practice including assessment of nutritional status, planning, implementation, and evaluation of interventions for patients over the age of one and across the lifespan
- Provides comprehensive nutritional education for a wide range of chronic and/ or complex health conditions using motivational interviewing techniques.
- Work collaboratively with a team of inter-professional health care providers to provide both direct and indirect care.
- Provide clear, concise, and timely documentation or case consult to the referring provider.
- Collaborate with PCN member providers, staff, and community stake holders to strengthen, develop and facilitate a range of programs and services that support healthy nutrition in CRPCN communities across the catchment area.
- Actively provide nutritional expertise, mentorship, and collaborative consultation to interprofessional team members and providers on an as-requested basis.
- Attend relevant community and professional meetings.
- Group class facilitation- within CRPCN and Community
- Engage with internal and external stakeholders (ex: AHS, PCNs etc) as appropriate.
- Work to achieve own professional development goals.
- Other responsibilities as required.
Educational Requirements:
- Baccalaureate degree with a Major in Nutrition Science/Dietetics/Foods and Nutrition
- Membership in good standing with the College of Dietitians of Alberta
Experience and Skills Required:
- 3-5 or more years of RD experience, preferably in a primary care setting.
- Knowledge of and interest in principles of a population-needs based approach, primary care, primary health care, and the medical home model
- Experience facilitating group classes would be an asset
- Experience with EMRs would be an asset
- Experience with disordered eating would be an asset

- Experience in leading program development and delivery.
- Excellent collaboration and high level of comfortability working in a team based care model.
- Ability to manage a varied and demanding caseload.
Working Conditions:
- Medical clinic and/or office environment
- Ability to work independently and with limited supervision
- Occasional travel for training or to CRPCN work sites across the catchment area required
- Valid driver’s license & access to reliable vehicle
